Credentialing time Frame: 30-60 days (Temps avail with a clear background/malpractice history)
License Requirements: Active, unrestricted license
Certificates must be AHA issued
DEA/CSR required
Hours: 7a-3p
Call Schedule: 3p-7a; every 3rd night; every 3rd weekend
Call Response: 30 minutes
Any Supervision: No
Model: All CRNA practice
Cases: Ortho, GI, OB/GYN, Cataracts, General, Peds (1 yr and up)
Spinals, Blocks and Epidurals Required: Yes
Case logs Required: Yes - proof of epidurals, blocks
Number of Providers: 3
Number of OR's: 3
